Wilderness Protocol Form

In this YouTube video Jerry, KG6HQD, discussed the Wilderness Protocol as used by hams. He also talks about a Search and Rescue form that he was offering for free on his website that can be used to leave with a responsible person when hiking or backpacking in the back country. I was unable to access Jerry’s website as it appeared to be down, so I regenerated the form from the screenshot in the video for my own personal use. Jerry was kind enough to give permission for me to post them here. There are PDF and Microsoft Word formats.
